All Health Forms Due for Geronimo and Summer Trip

Parts A, B, and C are required for Camp Geronimo and the Summer trip.  Part C requires a physical examination and signature from a physician.

Please be sure to have your son sign Part A.

The Immunization section must be filled out. We cannot accept separate immunization records. Waivers are available (Immunization Waiver Form)

• Keep the hardcopy for yourself.
• After scanning, send a copy to your personal email address.
• Be sure your son signs Part A.
• Be sure a parent or guardian signs Part A.
